AGENT AUTHORIZATION FORM
Customers / patients of Perfect Health Technologies (dba: Diet Doc Weight Loss) authorize Diet Doc Weight Loss to act as the customer/patients agent and receive medication prescribed by the doctor. This customer/patient prescription medication will be mailed to the patient the same day or no later than 1 business day after receipt of said medication. The medication is normally placed in a box containing non-prescription Diet Doc support products.
Perfect Health Technologies, Inc. DBA Diet Doc Weight Loss provides weekly follow up calls from our weight loss coaches during the first 3 months of the program. During this time, expect an automated dialer to phone you, instructing you to press "1" to speak to a weight loss coach if you are experiencing weight loss slow downs, stalls or need to re-order medication or product. Please sign to authorizing Diet Doc Weight Loss to contact you via phone, including using an auto dialer which connects to a live, weight loss coach.
RETURN POLICY & INSTRUCTIONS
Perfect Health Technologies – DBA: Diet Doc Weight Loss does not issue refunds for those who did not experience what they believe to be a satisfactory amount of weight loss during the weight loss phase of the Diet Doc Weight Loss Program. We cannot be responsible for those individuals who fail to follow our instructions.
However, we will accept refunds for unused (non prescription) products. Returning prescriptions is not prohibited. Non-used product must be received by our office prior to any refunds issued.

NOTE: If you are unhappy for whatever reason, and that is why you want to request a refund, our friendly and courteous support staff is standing by to resolve your issue. 9 times out of 10 we can help. Please include a completed Return Authorization form with all requested information, to be 100% sure your return is credited back to you.
PLEASE use tracking on your return shipment! Packages are lost more often than you might imagine, and if yours is lost and you cannot provide a tracking number to show that you really did send it back, we cannot process your return.
All products returns must be received by us no later than 30 days from your original purchase date. This policy exists because you received all material from us, including PDF doctor-written workbooks, cookbooks and other free materials and/or a doctor’s consultation. If we receive it after 30 days, we will still refund your charge, but a 15% processing & restocking fee will apply.
We do not reimburse shipping on returned merchandise nor do we provide return shipping labels.
All shipping charges are non-refundable.
Orders cannot be cancelled once they have been placed. This is because, in order to ensure the fastest shipping time, all orders are automatically transmitted to shipping department and the weight loss support products and prescription medications are sent out same day (after the initial doctor consultation) for shipment. If you have placed an order that you wish to cancel, simply return the products (unused) to us per the above instructions.
Refunds can only be issued to the credit card originally used to purchase. We do not issue refund checks - no exceptions! We cannot issue a refund if your credit card used to make the original purchase has been cancelled or has expired.
This policy is in place to protect us from the increasingly common criminal act of receiving a refund check, then disputing the original charge in order to fraudulently obtain two refunds from us. Unfortunately, this is becoming a common crime committed against internet merchants; therefore we no longer issue checks.

Also, if you are requesting a refund because you did not receive your product, please contact us first so we can track the shipment. 90% of "missing" orders turn out to have been signed for by the receptionist in an office or by a family member in a home who simply forgot to give it to you.
While we happily accept debit cards for payment, we suggest the use of a credit card instead to avoid any of these potential problems with debit cards.

CREDIT CARD CHARGEBACK POLICY
A "chargeback" occurs when a customer disputes a credit card charge. A fraudulent chargeback occurs when a customer attempts to dispute a charge for merchandise that they have actually purchased.
Attempting to dispute a valid credit card charge is fraudulent and is illegal. PERFECT HEALTH TECHNOLOGIES, INC. does not tolerate chargeback fraud, and we follow the following procedure when this takes place:
1. If you attempt to dispute or deny a valid charge, we will first contact you directly, because most disputed charges occur as a result of the customer not recognizing our charge on their statement. You will have ten (10) calendar days to reverse your dispute upon receipt of our letter.
2. If you do not reverse the dispute or otherwise attempt to resolve the situation, we will file a police report with your local authorities and seek criminal charges against you.
3. Your account will be turned over to a collection agency, which will initiate collection activities as well as report your account as delinquent to all of the major credit bureaus. In addition to our U.S. collection agency, we also contract with collection agencies abroad.
4. If you still do not resolve the situation by either reversing the dispute or reimbursing us for the amount disputed plus our bank fees, we will file a civil lawsuit against you for the amount disputed, for bank fees related to the chargeback, for additional damages for the harm your chargeback causes to our merchant credit record, and for punitive damages as the courts do not take credit card fraud lightly.

Packages are lost more often than you might imagine, and if yours is lost and you cannot provide a tracking number to show that you really did send it back, we cannot process your return. - DO NOT use an economy service like Media Mail or Parcel Post, because they can take up to two weeks or even longer and you may miss your 30-day trial deadline.
All 30-Day Free Trial returns must be received by us no later than 30 days from your original purchase date. This policy exists because you received instant access via PDF doctor-written and proprietary weight loss workbooks and cookbooks and were given information on the proprietary Diet Doc Weight Loss Program the same day you originally paid.
If we receive it after 30 days, we will still refund your charge, but a 15% processing & restocking fee will apply.
